Main duties of the job
To be responsible for the planning and delivery of occupational therapy and multidisciplinary assessments and interventions to patients within the falls prevention/ rehabilitation services prioritising clinical needs to provide an effective service.
To undertake occupational therapy assessments, for a designated caseload, addressing occupational performance, impairments identified and enabling the client in areas of self-care, productivity, and leisure.
To work with a caseload of patients of varying complexities and needs. To independently carry out rehabilitation programmes and interventions aimed at improving or maintaining independence. To formulate intervention plans which are aimed at maximising independence, safety, reducing the falls risks and reflect the clients' own goals. To plan and implement individual and/or group interventions in collaboration with the client using graded activity to achieve therapeutic goals. To undertake assessments of clients' home environments, identifying any modifications to the environment or assistive technology to support independence and safety. To undertake non-complex moving and handling assessments, multifactorial falls risk assessments prescribing equipment where necessary and handing over to clients and their carers in the safe use of the equipment.

Qualifications
Essential Diploma/Degree in Occupational Therapy. Registration with the Health and Care Professions Council.
Desirable Relevant post graduate training. Member of RCOT/special interest group/s.
Experience
Essential Application of the OT Process in a clinical setting. Experience of working as part of an MDT. Use of functional assessment and activity analysis. Relevant work experience in a variety of clinical placements.
Desirable Experience of delegation to support staff.
Skills
Essential Knowledge of current best practice in OT. Effective written and oral communication skills. IT Skills including Word processing and internet skills.
Desirable Knowledge and understanding of National Service Framework for Older people. Knowledge and understanding of NICE and COT Falls Guidelines.
