Job summary An amazing opportunity has arisen to join a small team of midwives to care for a caseload of women and families with socially complex needs; planning and implementing individualised midwifery led care in partnership with the woman and the multidisciplinary team. You will be working alongside multi professional teams providing personalised care for women whilst breaking down barriers that can inhibit families with socially complex lives accessing high quality maternity care. Main duties of the job To provide evidence based care for women and families with socially complex needs; planning and implementing individualised midwifery led care in partnership with the woman and the multidisciplinary team. Work within the community midwifery teams caring specifically for women and families who have socially complex and/or safeguarding needs during the antenatal and postnatal period. Work closely with the Safeguarding Team, as well as the wider multi-disciplinary and inter-agency teams. Provide antenatal and postpartum care reporting and deviations from the normal to a more experienced midwife, and respond appropriately to all emergency situations. Encourage a holistic, non-stigmatizing approach to socially complex women and their families. Work in partnership with other health care professionals and liaise closely with other agencies involved in the provisions of maternity services and seek advice from the Safeguarding Midwives/Team Leader as required. Recognise that the vulnerability factors can be interrelated and that parental factors increase vulnerability to child abuse and neglect. Work closely with social care and safeguarding team in care planning and assessment of women and families. Work closely with Early Help agencies and ensure that women and families are linked into additional support at the earliest opportunity. Contribute to pre-birth assessments and case conferences ensuring effective sharing of information. Work closely with the Community Team Leader, Safeguarding Midwives, Woman's Lead Consultant Obstetrician, and Consultant Midwife for Safeguarding and Public Health. Attend monthly internal Psychosocial/safeguarding meetings to give and receive updates on cases. Attend case conferences, core group meetings and any other meetings required for the case-load. Ensure that robust pre-birth/discharge planning and plan is in place prior to birth in conjunction with the Safeguarding Team. Ensure robust information sharing and care planning with primary care teams. Attend Postnatal Discharge planning meetings on the postnatal ward for any cases with social care involvement. Education & Qualifications Essential Registered Midwife First degree in Midwifery or equivalent experience Desirable Evidence of post qualification education activity Mentorship qualification Evidence of safeguarding post qualification education Experience Essential Completion of a preceptorship programme or equivalent, with evidence of working in a variety of settings Evidence of applying current research to practice Relevant clinical experience Excellent organisational skills Desirable Managerial experience
