We are looking to recruit motivated and enthusiastic GP(s), ideally for 4-8 sessions a week. Our practice employs a wide MDT that shares the GP workload and currently comprises our experienced GP Partners/Clinical Directors, Managing Partner, Nurse Manager, Advanced Nurse Practitioners, First Contact Physio, Chronic Disease Specialist Nurses, Social Prescribers, Nurses, Pharmacists, and Health Care Assistants, who work across our practices. We work hand in hand and have excellent working relationships with our community and social care providers, having well-established multi-disciplinary team meetings covering safeguarding, frailty, dementia, and palliative care. We also work very successfully with third sector partners and have an extremely active Patient Participation Group with whom we work very closely. We hope to start training GPs and/or F2 doctors at the practice, so an interest in training would be welcome, and we are currently actively supporting trainees from several disciplines including physician associate students, nurses and nursing associates, as well as pharmacists. THE ROLE The purpose of the role is to manage a clinical caseload and deal with a wide and diverse range of health needs, ensuring the highest standards of care for all registered and temporary patients. The role may include additional clinical responsibility based on the post holder’s interests and skills. Willingness to attend Clinical, Safeguarding, and Meds Management meetings may also be required. MAIN DUTIES OF THE JOB: The post holder will make themselves available to undertake a variety of duties including surgery consultation, telephone consultations and queries, visiting patients at home, checking, and signing repeat prescriptions, and dealing with queries and clinical paperwork. Making professional autonomous decisions in relation to presenting problems whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. Developing care plans for health in consultation with patients in line with current practice disease management protocols. To assess, plan, monitor and evaluate patient care using evidence-based practice and in accordance with practice protocols and guidelines. To establish and monitor good working relationships with the GPs and other health care professionals, statutory and voluntary organisations. PERSON SPECIFICATION: Essential: Full GMC Registration, MBBS (Bachelor of Medicine qualification), vocational or additional experience in general practice, commitment to professional development, knowledge of the NHS, and full immunisation record. Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
