An exciting opportunity has arisen within Hampshire & Isle of Wight Foundation Trust (HIOW) to appoint an inspirational and enthusiastic qualified Nurse or Midwife to work within the School Nursing Service. Portsmouth Health Visitors/School Nurses sit within an integrated 0-19 service with the Local Authority and work with the wider multi-agency and multi-disciplinary children's workforce to identify need and provide timely intervention at the earliest opportunity. Health visitors/School Nurses are well supported to achieve this through a developing 'Team Around the Worker' model, utilising consultation with other specialist practitioners working across the 0-19 service in areas such as perinatal infant mental health, substance misuse, domestic abuse and children with disabilities. A full driving licence and use of a vehicle is essential. Main duties of the job

Identify health needs by implementing the Healthy Child Programme in accordance with School Nursing policies and guidelines. Provide information, advice and support for young people, staff and parents on general and specialist health issues. Work closely with school staff to advise on Health Education / Health Promotion within the scope of the National Curriculum. Provide information, advice and support on emotional and personal problems to young people and staff via the referral system to the Service. Update school staff in the following: Asthma and anaphylaxis. Monitor children at risk and liaise with school staff, parents and other agencies in planning and implementing care. Facilitate the implementation of local / national agreed health promotion / education strategies. Comply with NMC Code and other related documents e.g. Record keeping, Administration of Medicines, Confidentiality and Scope of Professional Practice. Raise awareness of the need for vaccination against preventable diseases through education of young people including Staff, Parents. Be responsible in organising the delivery of school vaccination sessions under the supervision of a senior nurse. To undertake a Health Needs assessment with the family and refer on as per care pathway ensuring correct medical referrals and support is in place where medical needs are identified. About us

Salary: £29,970 to £36,483 a year based on full time hours.
