The role holder will assist individuals, carers and families to develop support plans to meet the outcomes identified, using a strength-based, asset-based approach and community options where appropriate.
Mental Health:
Experience of mental health social work is preferred. There are opportunities to train as Approved Mental Health Professionals (AMHPs) after a qualifying period.
Safeguarding:
You will need experience of adult safeguarding, knowledge of relevant legislation, and making safeguarding personal.
Learning Disability:
To provide a professional social work service to adults with a mental health condition, their families, and carers who are residents of Hammersmith and Fulham in line with the requirements of the Care Act 2014.
The post holder will ensure that care and support are person-centred and put the people with whom they work in control of their lives. They will work within legislative frameworks, including s.9, s.13, and s.42 of the Care Act 2014, NHS Continuing Healthcare, and Community Deprivation of Liberty considerations.
We have a hybrid working model, and staff will be supported with mobile IT equipment and hot desk facilities in all offices across the borough. Professional, practice-based supervision will be provided.
