To provide a professional social work service to adults with a mental health condition, their families, and carers who are ordinary residents of Hammersmith and Fulham in line with the requirements of the Care Act 2014. The post holder will ensure that care and support are person-centred and put the people with whom they work in control of their lives. They will work within the legislative frameworks, including s.9, s.13, and s.42 of the Care Act 2014, NHS Continuing Healthcare, and Community deprivation of liberty considerations. We have a hybrid working model, and staff will be supported to do this with mobile IT equipment and hot desk facilities in all offices across the borough.
