The post holder will work as an autonomous nursing practitioner being a named nurse for a designated caseload for CYP and families. The post holder will work collaboratively with other professionals and agencies in the assessment, implementation, evaluating specialist care to children with long-term, life-limiting or palliative care conditions. To provide and promote evidenced-based knowledge and skills to manage the health outcomes for CYP and their families and carers across a variety of community settings to safely meet the CYP health care needs. To act in an advisory role to other professionals and staff from other agencies in respect of health care for CYP and families within their area of specialism. The post holder will support the wider ICPS and participate in the service's provision for telephone 24-hour-on-call and overnight home visits to support end-of-life care when required. To support and contribute to the further development of the Integrated Community Paediatric Service (ICPS), helping to ensure that policy and practice is developed and delivered in line with Trust, regional, and national best practice. Main duties of the job

As a senior Children's Community Nurse within the ICPS, you will provide effective leadership across the service around issues within your area of specialism and scope of practice. To work in partnership with other health professionals and agencies to safeguard children and young people. Act as the CYP and their families in advocating, promoting, and delivering the highest standard of evidenced-based care. To provide clinical knowledge to assess, identify, prioritise, implement, and evaluate health assessments, programmes of care, and provide advice to the other professionals, CYP, and their families and non-medical carers. The clinical responsibilities provided must be delivered within the professional area of expertise, i.e., paediatric nursing. Promote continuous improvement for the CYP and their families by collating their experience. Communicate effectively with a wide range of people including the young people and their families, colleagues from health, education, and social care, and other relevant professionals. Provide line management responsibility for junior staff, i.e., Children's Community nurses/ Assistant Practitioners/ Health Care Assistants. Promote a positive learning environment within the service for all staff and visiting students by advising on professional development; ensuring supportive preceptorship and providing adequate mentorship for those in training. Qualifications

Educated to degree level Specialist practitioner children's community nursing Experience

Evidence of recent professional development Evidence of a wide range of experience across paediatric nursing. Example: Acute paediatrics and CYP with long-term conditions, Tracheostomy care Evidence of multi-agency working Demonstrate evidence of clinical risk Skills

Effective communicator to a wide range of professionals and situations Demonstrates the ability to prioritise, implement and use new ways of working. Knowledge

Underpinning knowledge on a wide range of clinical skills. Example: Ventilation, suctioning, tracheostomy care, ANTT, IV therapy, BLS Understanding of the wider workforce across community providing clinical support to CYP. Understanding of audits Other

Flexible working to meet the needs of the service and CYP Able to travel across the community independently Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £37,338 to £44,962 a year per annum pro rata.
