Hampshire and Isle of Wight Healthcare (HIOWH) NHS Foundation Trust have exciting and new Medicines Management posts, that will be pivotal in supporting meaningful change in our Hospital at Home Frailty Virtual Wards (H@H VW). These roles include a pharmacist and pharmacy technician, working in partnership with consultant practitioners (Nurse and AHP's). We offer the appointee a supportive environment to develop their clinical skills. They will undergo a supernumerary induction period and be mentored and supported by senior pharmacy colleagues as well as the Clinical leads in H@H VW and UCR. The H@H VW supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, including intravenous therapies. The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. The post holder will be expected to work between two teams across Mid and North Hampshire, with team bases in Herriard, located in-between Basingstoke and Alton, and Colden Common in Winchester. A hybrid approach to working, with a mixture of remote and in-person clinical support will be considered for the right person. Main duties of the job

The post holder will be required to work autonomously to: Deliver a high-quality Medicines Optimisation Service to high-risk patients. Ensure an integrated approach to the management of medicines and continuity of pharmaceutical care for patients. Provide professional expertise, advice, support and training on policies and practice to health and social care professionals in the community. Conduct clinical medication reviews with patients in their own homes to ensure patients' medicines are optimised for them to gain the maximum health benefits by utilising relevant and current evidence-base and local/national guidance. Individualise treatment, taking into account the patient's ability/willingness to adhere to the prescribed medicines regimen. Make prescribing recommendations as per local and national guidance to the H@H VW and UCR team. Prescribe and deprescribe within their competence for patients to improve patient care. Provide expert pharmaceutical information and advice to the H@H VW and UCR multi-disciplinary teams and where applicable to others involved in a patient's care. Focus on outcomes delivering improved benefits and value to patients from their use of medicines, appliances, and devices. Oversee medicines supply, storage and security. Respond to unpredictable work patterns and urgent requests for help by phone or email within a reasonable time frame. About us

Qualifications

Undergraduate Degree in Pharmacy: MPharm Registered as a pharmacist with the General Pharmaceutical Council (GPhC) or equivalent prior to 2002 At least 5 years relevant post-registration experience. Specialist knowledge acquired through a post graduate diploma course in Clinical Pharmacy or similar or alternatively acquired through relevant and equivalent experience Registered to be an independent prescriber with the GPhC or working towards this qualification Master's degree in pharmacy or equivalent. Post-qualification experience of clinical pharmacy, medicines information and prescribing advice Experience of working in a multidisciplinary environment Broad specialist clinical pharmacy knowledge. Experience of audit techniques Understanding of issues related to independent prescribing. Involved in identifying and assessing risk to improve patient safety. Expertise of addressing and resolving legal issues related to medicines. Ability to work under pressure and be able to make important decisions in pressured situations. Salary: £53,775 to £60,504 a year based on full-time hours.
