We are looking to recruit an experienced Senior Clinical Practitioner to join our Rough Sleeping and Mental Health Project team.
The RAMHP team is a Southwest London & St Georges Mental Health (SWLSTG) service that aims to improve the mental health of people sleeping rough in the five boroughs which make up SWLSTG. The team has close operational working links with other advocacy and support services in the non-profit sector and enhances, fills gaps, builds bridges between services, and helps existing services work together more effectively so that homeless people are rapidly identified, assessed, supported, and directed so that their recovery is sustained. RAMHP is designed to complement, strengthen, and enhance existing provision.
This post is funded by NHS England's Longer Term Plan funding stream. The RAMHP project fits the priorities for the Greater London Authority's action plan for homelessness by working to: Minimise the flow of new rough sleepers onto the streets Ensure that no one new to the streets sleeps rough for a second night Ensure no one returns to sleep on the streets of London Tackle rough sleeping by non-UK nationals Improve partnership working around enforcement Improve outcomes for people discharged from hospital or prison Meet the physical and mental health needs of rough sleepers Improve access to mental health services Ensure the availability of appropriate accommodation, including emergency accommodation Main duties of the job
The post holder will work with people who are rough sleepers experiencing mental health problems who may have co-occurring alcohol and drug problems. They will provide assessment and link in with other advocacy and support services in the non-profit sector as well as helping existing services to work together effectively so that homeless people are identified, assessed, supported, and directed so their recovery is sustained.
The successful candidate will need to demonstrate excellent interpersonal skills, an aptitude for managing multiple demands and effective time management techniques whilst remaining calm under pressure. A positive outlook and ability to motivate others is also desirable.
You would need to have excellent communication skills, be proactive, able to work autonomously and in complex environments. Interested candidates would be expected to think creatively and innovatively. You will also be flexible, responsive, and organised, with an interest in developing services and participating in service modernisation.

Job responsibilities
To act as duty for the team on a regular basis overseeing referrals and managing the team's response to requests for urgent assessment. To promote and develop multi-disciplinary working within the team ensuring that respect is fostered between the disciplines. To manage a caseload of rough sleepers with complex needs as agreed with line manager and within the service acceptance criteria. To conduct assessment, liaison, and brief intervention, ensuring that lines of communication are effective and regular reviews are in place. To keep accurate up-to-date records of all contacts with service users and communications with other professionals involved in their care ensuring that electronic records are updated regularly and distributed as appropriate. To maintain and develop collaborative working relationships with service providers from a wide range of agencies. To develop and maintain a high standard of practice based on a comprehensive assessment using a range of psychosocial and clinical approaches. To establish joint and/or co-working practice for clients with other team members and/or external service providers as appropriate. To fully participate in the assessment and care of rough sleepers with mental health needs, implementing appropriate interventions with them and their carers, based on up-to-date theory, the practice model of the service, and relevant other guidance. To undertake risk assessments with team members and health professionals from other agencies, recording and sharing information as necessary and within confidentiality guidelines, in a manner which is timely to ensure the safety of the service user and others. To visit rough sleepers in an environment that suits them, on the street, day centre, work, and recreational centres (as appropriate) to engage and encourage participation in treatment and in all aspects of daily living. To liaise closely and attend meetings and ensure colleagues from voluntary organisations are invited and attend relevant meetings. All parts of the system are communicating. To joint work with team members and colleagues from statutory/non-statutory agencies, either through direct client contact or in a clinical consultation capacity. To provide information to assist service users in making informed choices regarding opportunities and services available to them and to assist them in accessing the required services, support, and entitlement. To involve families and carers in treatment plans where appropriate and with the service user's consent. This may require direct family interventions.
