Frimley Health Foundation Trust has an exciting vacancy for a Band 6 Senior Community Dietitian on a part-time basis of 22.5 hours per week. Flexible work options can be considered. The responsibilities within this role include delivering a dietetic service to housebound patients and Care Home residents at risk of malnutrition in the Frimley South area. The post holder will work closely with the wider relevant multi-disciplinary teams and provide support to the Medicine Optimisation team. You will lead on various projects such as appropriate prescribing of Sip Feeds and Quality Improvement projects in care homes. Our department is a well-established team of dietitians working across various specialties. To provide individualised nutrition assessments and reviews for patients requiring oral nutrition support in care homes and those who are housebound across Frimley South Place. To be a source of expertise for FHFT and Frimley ICB on issues relating to oral nutrition support in the community. To support the team lead and Frimley ICB medicines optimisation team in ensuring appropriate use of oral nutritional supplements in the community to treat malnutrition. To educate, train, and disseminate information to health and social care professionals including clinicians, nurses, allied health professionals, dietetic colleagues, care home staff, and students about the dietetic management of malnutrition in the community. Qualifications

Recognised degree or postgraduate diploma in nutrition and dietetics or equivalent. HCPC registration. Evidence of CPD. Nutrition-based course post-registration. Student supervision training. Member of BDA. Experience

Post-registration clinical dietetic experience including community oral nutrition support. Training and education of healthcare professionals and patients. Multidisciplinary team working. Leadership. Research. Skills

Organisational and prioritisation skills. Problem-solving. Disclosure and Barring Service Check
