To provide individualised nutrition assessments and reviews for patients requiring oral nutrition support in care homes and those who are housebound across Frimley South Place. To be a source of expertise for FHFT and Frimley ICB on issues relating to oral nutrition support in the community. To support the team lead and Frimley ICB medicines optimisation team in ensuring appropriate use of oral nutritional supplements in the community to treat malnutrition. To educate, train, and disseminate information to health and social care professionals including clinicians, nurses, allied health professionals, dietetic colleagues, care home staff, and students about the dietetic management of malnutrition in the community.
