6 Senior Community Mental Health Nurse

post within our Adult Community Mental Health Team covering Newmarket and surrounding areas. This is a fantastic opportunity for practitioners motivated to advance their career. If you are currently in a Band 5/6 position and have at least 12 months post-registration experience, this may be the post for you. As a service, we pride ourselves on supporting our clinicians to develop in a range of areas. Main Duties of the Job

The post holder will undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary team (MDT)/multi-agency environment. Managing a caseload and providing mental health care services to individuals with mental health needs. Conducting planned care/risk assessment/screening for patients within their care plan and ongoing evaluation of patients’ needs. Initiating and attending patient-centered reviews, case conferences, and other meetings as required. Advising the Responsible Medical Officer(s) and/or health and social care professionals of any changes in patients’ conditions. Working within the MDT and forming partnerships with other care providers. Empowering service users/carers to make informed decisions about their care. Providing caseload management supervision to junior staff under the direction of the CTL. Assisting with the supervision of peers if requested. Enabling patients to gain and maintain independence. Identifying personal learning and training needs through KSF, appraisal, and personal development plans. Supervising students undertaking placements within the Team. Person Specification

Experience

Experience working within a multi-disciplinary team. Experience delivering person-centered treatment packages. Experience in mental health. Community experience including care coordination. Qualifications

Willingness to train to become a Sign-off Mentor. Knowledge

Knowledge of medication and monitoring its effects. Knowledge of psychosocial interventions. Developing knowledge of local systems and networks. Other
