You will work as part of an integrated multi-disciplinary team made up of Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health Workers, Social Workers, and Administrators.

The role requires working independently and autonomously with service users with serious mental illness, supported by qualified mental health nurses and the multi-disciplinary team.

You will be expected to work across the service in the community, enabling and empowering individuals to access opportunities that promote their recovery. Under the direction of registered/qualified staff, you will assist in the provision of clinical care for adults with severe mental illness.

Key Responsibilities:

Promote patients' recovery in the community while adhering to legislation, policy, and procedures. Provide support to an allocated group of patients, promoting their recovery and social inclusion to maintain them in their community environment (STR role). Focus on the direct needs of patients, working across boundaries of care, organization, and role, coordinated through the Integrated Care Programme Approach (ICPA). Support patients by participating in the assessment, planning, and implementation of their care plan, including telephone, digital, and face-to-face contact.

This is a fixed-term role, and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.

Previous experience of working within Mental Health would be advantageous, along with a flexible, innovative, and person-centered approach to practice.

A car owner/driver is essential, subject to the provisions of the Equality Act (2010).

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.
