An exciting and unique opportunity has arisen for an enthusiastic, flexible and highly motivated band 6 Physiotherapist to join the Eden ICC team. Formally known as the Community Rehabilitation Service, we now provide the opportunity to work within a fast-paced and dynamic service both developing a rapid response element to the team alongside providing rehabilitation across Eden ICC. Eden ICC provides admission prevention, supported discharge, specialist assessment, and rehabilitation for patients with a wide range of long and short-term conditions. We work closely with our health and social care colleagues for care and advice. The successful candidate will need to be motivated and able to demonstrate excellent assessment and communication skills as well as good team working. Main duties of the job Eden ICC demonstrates fantastic multidisciplinary team working by supporting D2A's (Discharge to Assess) and providing rehabilitation alongside rapid response care. Patients can be seen as in-patients (in Community Hospital beds) but the majority of work is based in patients' own homes (including residential and nursing homes).
