We are currently seeking a Senior Community Physiotherapist/Occupational Therapist to join our dedicated Neuro Rehabilitation Team in South Yorkshire. The organisation is committed to providing high-quality healthcare services to individuals across South Yorkshire, focusing on a holistic approach to care that enhances the independence and well-being of service users through personalised rehabilitation programmes. Role Overview:

As a Band 6 Community Physiotherapist/Occupational Therapist, you will be responsible for delivering comprehensive rehabilitation services to individuals within their own homes and community settings. You will work collaboratively with a multidisciplinary team to assess, plan, and implement effective treatment plans tailored to the unique needs of each client. Key Responsibilities: Conduct thorough assessments of clients’ physical and functional abilities. Develop and implement individualised treatment plans, including therapeutic exercises, mobility training, and adaptive equipment prescription. Provide education and support to clients and their families regarding self-management techniques and strategies for optimising independence. Collaborate closely with other healthcare professionals, including nurses, social workers, and community support workers, to ensure coordinated care delivery. Maintain accurate and up-to-date clinical records in accordance with organisational policies and professional standards. Requirements: Degree in Physiotherapy or Occupational Therapy from a recognised institution. Registration with the Health and Care Professions Council (HCPC). Minimum of two years’ experience working as a Physiotherapist or Occupational Therapist in a community setting. Strong clinical assessment and treatment planning skills. Excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds. Commitment to continuous professional development and evidence-based practice.
