Duties and Responsibilities

Communication and Working Relationship Skills

Contribute to the team's engagement with a wide range of stakeholders including staff, service users, their carers, and other statutory and voluntary sector agencies.

Impart complex and sensitive information to:

Patients and families/carers regarding assessment, care planning, treatment, and review.

Multi-disciplinary teams to ensure the provision of consistent, well-coordinated care.

Primary care teams regarding client care.

External agencies involved in the provision of care (e.g., voluntary and private sector, police, social services, housing, employment).

Service user/carer groups.

Liaise with a wide range of other professionals and agencies.

Contribute to the safe and effective day-to-day operations of the service, including handling clinical inquiries.

Analytical and Judgemental Skills

Practice in a way that actively minimizes dependency and promotes recovery.

Communicate information in a relevant and understandable way for service users and carers, adhering to the principles of policy implementation guidelines.

Aim to uphold the principles and approach of CPA, regardless of formal CPA status.

Work cooperatively with staff colleagues, accepting recent assessments of clients to minimize disruption.

Ensure a duty of care to patients and clients, considering their wishes, feelings, values, and beliefs.

Take personal accountability for your own practice and ensure professional competence through clinical supervision.

Planning and Organisational Skills

Attend management and professional meetings as directed by the line manager and professional lead.

Plan and coordinate own workload in collaboration with service user needs.

Plan and organize case conferences, liaising with other agencies as required.

Ensure interventions and care plans are developed in an integrated and coordinated manner.

Responsibility for Patient/Client Care, Treatment & Therapy

Maintain a caseload of appropriate clients and develop care packages including assessment, planning, intervention, and evaluation.

Triage referrals by assessing and advising on eligibility criteria.

Provide specialist advice to referrers and other multi-disciplinary staff regarding client care.

Participate in reviews in accordance with Care Programme Approach and ensure completion of relevant CPA documentation.

Promote medication concordance and administer medication as required according to relevant policies.

Participate in safeguarding children to level 3 as set out in Safeguarding children and young people roles and competencies for Health Care staff.

Coordinate care for individuals on the caseload where appropriate.

Responsibility for Policy and Service Development Implementation

Participate in audit and research programmes within the team and implement findings.

Contribute to professional clinical governance arrangements to ensure service quality.

Participate in the development of Specialist Learning Disability Services.

Responsibility for Finance, Equipment, and Other Resources

Responsible for care of own equipment and handling or processing cash or patient valuables where appropriate.

Responsibility for Human Resources

Provide information, education, and advice to carers, families, and providers.

Supervise junior qualified staff in triage and assessment processes.

Provide clinical supervision for staff and coordinate team activities.

Mentor students allocated to the team and take clinical lead in the absence of the team leader.

Responsibility for Information Resources and Administrative Duties

Ensure timely, accurate, clear, and concise records are maintained in accordance with trust policy.

Provide supervision and leadership for band 3/4/5 staff where appropriate.

Responsibility for Research and Development

Assist with research programmes within the Trust, service, and team when appropriate.

Freedom to Act

Work under the direction of the team manager but able to work autonomously using own initiative.

Responsible for fulfilling responsibilities as outlined in the job description and professionally accountable for all actions undertaken.

Any Other Specific Tasks Required

Comply with any reasonable request from the team manager or other senior clinical staff.
