Job summary When working in the HTT team, practitioners undertake assessments and treatment as required. The person will lead in the assessment, co-ordination, planning, implementation, evaluation and delivery of high quality home treatment for people in acute Mental Health crisis, with the aim of promoting their recovery and maintaining them in a community setting rather than in hospital. As a band 6 practitioner, you will operate within a community setting and work within a multi-disciplinary team which aims to reduce hospital admissions and facilitate early discharge through home-based treatment. You will also work closely with inpatient and community services. The ability to travel to meet the requirements of the role is essential to undertake visits across the North Devon locality. Main duties of the job Undertaking assessments in various settings to include the emergency department and people's homes. Triage referrals from various sources such as Liaison psychiatry, AFRS and community Keyworkers. Liaising with other services such as drug and alcohol service, housing, citizens advice, NDADA and social services. Creating personalised care plans and dynamic risk assessments to include safety planning. Engaging with families and carers and signposting to relevant support. Ability to work independently, adhering to the lone working policy and working as part of a team. Ability to work as the shift coordinator on a rota basis. To work as part of a large multi-disciplinary team whose remit involves Crisis and Home Treatment interventions to support people to remain at home during a mental health crisis, as well as facilitate appropriate step down from acute psychiatric admission. Experience Essential Substantial experience of managing a busy caseload of clients with a range of mental health problems. Experience of working in a multi-disciplinary and multi-agency environment. Providing clinical supervision. Assessment experience, particularly mental health and managing risk. Experience of working with NHS IT systems and keeping clinical documentation up to date. Qualifications Essential A professional mental health qualification e.g., RMN, CQSW / ASW, BSc/Diploma in OT. A current registration with a professional body i.e., NMC.
