The role involves;assessing eligibility criteria under the NHS National Framework for ContinuingHealthcare and Funded Nursing Care 2009 (revised 2022), planning andcommissioning packages of care (including Personal Health Budgets), supportingindividuals, their families and health professionals through the ContinuingHealthcare process (including transition from child to adult services) andreviewing eligibility/packages of care.

Main duties of the job
We need a nurse practitioner who is experienced working with people who have learning disabilities - essential characteristic. NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs. Devon Partnership NHS Trust is responsible for the decision making and commissioning processes, working closely with the Local Authority's, on behalf of Devon ICB. Successfulcandidates will have excellent communication and assessment skills; be resilient,able to work autonomously but also work as part of a team. To monitor procurement of costeffective services Responsible for reviewing a personseligibility for Continuing Healthcare Funding and determining on goingeligibility for CHC / FNC ensuring that the national framework is applied andadheres to all local policies and procedures. To monitor and review quality of careand support arrangements to ensure person is being safely supported in anenvironment which continues to meet their needs. To support discharge planning fromHospital and commissioning of future package of care and support. Responsible for Case Management ofclients on Section 117 Aftercare, where required, as determined by DPT / DCClocal policy.
