The role involves;assessing eligibility criteria under the NHS National Framework for ContinuingHealthcare and Funded Nursing Care 2009 (revised 2022), planning andcommissioning packages of care (including Personal Health Budgets), supportingindividuals, their families and health professionals through the ContinuingHealthcare process (including transition from child to adult services) andreviewing eligibility/packages of care. We need a nurse practitioner who is experienced working with people who have learning disabilities - essential characteristic. NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs. Devon Partnership NHS Trust is responsible for the decision making and commissioning processes, working closely with the Local Authority's, on behalf of Devon ICB. Successfulcandidates will have excellent communication and assessment skills; be resilient,able to work autonomously but also work as part of a team. This is a communitybased post and although the role is based in Mid Devon, post holderswill be expected to be able to travel covering the whole of Devon, andoccasionally out of the county. The team values arole which is fast paced and employs the use of holistic nursing where we lookat the needs of the person. To actively engage with a wide rangeof stakeholders including staff, service users, their carers and otherstatutory and voluntary sector agencies to ensure people are fully briefed,know and understand commissioning and care management processes in relation toContinuing Healthcare, Funded Nursing Care, health and social care fundedpackages of care and section 117 aftercare. Impart complex and sensitive information to : Service user and families/carers re assessment, careplanning, treatment and review Multi-disciplinary teams to ensure the provision ofconsistent well-coordinated care Primary care teams regarding client care External agencies also involved in the provision ofcare e.g.: voluntary and private sector, police, social services, housing,employment etc. Safeguarding, DOLS teams, Clinical CommissioningGroups and Devon County Council regarding concerns arising from commissionedpackages of care. Liaise with a wide range of other professionals andagencies. To Liaise with Care Brokerage as appropriate. Contribute to the safe and effective day to dayoperations of the service including the handling of clinical enquires. Provide and receive information some of which maybe contentious, sensitive and complex to and from individuals and theirfamilies/carers, where there may be barriers to understanding. Provide high standard of written and verbalcommunication that is clear and in line with professional documentationstandards Participate in and chair clinical meetings, thiswill include multi-disciplinary meetings to agree DSTs and Section 117 reviews. Demonstrate the ability to communicate in a waythat is effective, empathic and reassuring whilst employing negotiatingtechniques to gain co-operation from highly complex individuals. Communicate information in a way that makes itrelevant and understandable for service users Analytical andJudgement skills Use clinical judgement to support theprocurement and commissioning of packages of care for adults with a learningdisability and complex health needs to ensure people are maintained withintheir local communities, avoid inappropriate admission to hospital andfacilitate timely discharge from inpatient settings. Use clinical judgement to considercomplex facts or situations, requiring analysis, interpretation and comparisonof a range of options when determining the type of care package required forsomeone with complex health needs. Give written / oral evidence tosupport rationale to assist verification and funding panels in determining theeligibility for NHS Continuing healthcare Planning and Organisational Skills Organise and facilitate complexmulti-disciplinary team meetings, these will include: DST review meetings andSection 117 aftercare meetings. Lead on the process for contentiousdecision making when other parties do not agree with outcomes with regard toeligibility for CHC or FNC funding. This may involve the wider MDT, serviceusers, relatives, carers and advocates including legal representatives Plan and organise individualspackages of care and ensure that reviews are held within the required timeframes, for example CHC, FNC and Section 117 annual reviews. Ensure care/support package meets theindividuals assessed needs and agreed outcomes and is appropriate to achievethe identified intended outcomes in the care plan Responsible for managing their ownworkload to ensure the needs to the individuals who use our services are mettaking the wider needs of the team into consideration. Ensure that interventions are plannedin an integrated and co-ordinated manner particularly where dependent on otherexternal agencies. Plan own workload and the workload ofjunior colleagues to ensure that care is provided in the most high quality andcost efficient manner, prioritise and make adjustments as appropriate. Responsibility for Individuals eligible for health funded care To be responsible for comprehensivehigh quality assessments to determine eligibility for Continuing HealthcareFunding in accordance with national framework. This will also include FundedNursing Care and health and social care joint funding. To support the tendering of allagreed packages of care that fully meets the needs of individuals with complexhealth needs. To monitor procurement of costeffective services Responsible for reviewing a personseligibility for Continuing Healthcare Funding and determining on goingeligibility for CHC / FNC ensuring that the national framework is applied andadheres to all local policies and procedures. To monitor and review quality of careand support arrangements to ensure person is being safely supported in anenvironment which continues to meet their needs. To support discharge planning fromHospital and commissioning of future package of care and support. Responsible for Case Management ofclients on Section 117 Aftercare, where required, as determined by DPT / DCClocal policy. To be responsible for reviewingcomplex packages of care in partnership with clients, carers andmulti-professional agencies and representatives. To lead on brokerage and procurementof alternative care and support for clients whose placements have broken down.To ensure funding and brokerage are completed within a timely manner to preventunnecessary hospital admission. To support safeguardinginvestigations by reviewing quality and provision of health funded packages ofcare where required. To support transition of young peoplewho are eligible for health funded packages of care when entering adulthood Responsibility for Policy and Service Development Implementation Be conversant with the vulnerableadult and child protection procedures and policies of the organisation To keep up to date with nursingpractice and research, along with guidance from Nursing and Midwifery Council. To be responsible for ownprofessional development and to keep up to date with current issues in theprofessional field Responsibility for Human Resources e.g. Supervision, training, HR adviceand Management The post holder will deputise for theCHC lead nurse as necessary in his / her absence To supervise and support work of Band5 staff in planning, prioritising and organising workload according to theneeds of the service. To supervise Band 5 staff includingyearly appraisal and professional development planning. There is a requirement to supportlearners on a regular basis which includes the achievement and updating ofprofession specific training to the required standard to fulfil this role. To act as a role model for otherstaff and students demonstrating high standards of practice and professionalconduct To deliver appropriate training andsubsequent support to other staff related to Continuing NHS Care To participate and facilitate clinicalsupervision To work within the standards set outby HCPC and N.M.C Code of Professional Practice To participate in and gatherinformation for safeguarding vulnerable adult processes as required. Any Other Specific Tasks Required Practice in a way that activelyminimises dependency and promotes recovery. Communicate information in a way thatmakes it relevant and understandable for service users and carers, working tothe principles of the functions policy implementation guidelines and practicein line with the standards and values set out in them. Ensure a duty of care to serviceusers who are entitled to receive safe and competent care. The service userswishes, feelings, values and beliefs should be taken into consideration as wellas the views of the people close to them. Take personal accountability for yourown practice. This means that you are answerable for your actions andomissions, regardless of advice or directions from another professional Ensure that you remain professionallycompetent by participating in your own and others clinical supervision. The ability to access all areas ofDevon in a timely manner is essential.
