The Community Recovery Service is an integrated provider with the London Borough of Hounslow offering Intermediate Care and Reablement for Hounslow residents. The team is comprised of Occupational Therapists, Physiotherapists, Nurses, and Rehabilitation support staff. The service delivers on a range of pathways including community rehabilitation, Reablement and community intermediate care following hospital discharge. The service operates 7 days per week and staff are required to work at weekends and on Bank Holidays as part of a rota. We are currently seeking a senior Occupational Therapist with a broad range of skills, experience and knowledge who is looking for an opportunity to further develop skills in community rehabilitation and Reablement. The post offers exciting opportunities to work within our forward-thinking service. You will be part of a large, supportive rehabilitation service, based at the Heart of Hounslow Centre for Health. Main duties of the job

We are seeking an Occupational Therapist with the experience and confidence to work autonomously within the community. You will be required to use your previous experience to work as part of a multidisciplinary team whilst using specialist skills to assess occupational performance and inform rehabilitation objectives, interventions and community treatment plans. You will bring a comprehensive understanding of the role of Occupational Therapy in a community setting and within the context of Intermediate Care. We are looking for someone who is enthusiastic and able to draw on their experience to deliver high quality clinical services to the population, whilst offering specialist input within the team. The service is delivered through multiple site clinics and home visits. The post holder will work flexibly and be mobile across the Hounslow Borough. About us

Qualifications

Diploma/Degree in Occupational Therapy or equivalent HCPC registered Evidence of personal / professional development (portfolio) Membership of RCOT Membership of relevant Clinical Interest Groups Experience

Completed a minimum of two years qualified Occupational Therapy experience Post graduate experience that covers core skills in rehabilitation including therapeutic handling and management of people with complex conditions Broad experience including orthopaedics, medical, neurology, intermediate care and elderly care Specific experience in speciality designated to post Experience of working as part of a multidisciplinary team Ability to contribute to clinical education of less experienced staff, students or assistants Involvement in audit, research and understanding quality issues Experience of supervision of junior staff, student or rehabilitation assistant Evidence of role in service management / development Experience of delegation to colleagues Knowledge

Well established knowledge of assessment tools and treatment techniques relevant to this client group Experience and knowledge of community and neuro-rehabilitation Occupational Therapy services Remain updated with professional practice and new research Understand the legal responsibilities of the profession Work within and understand national and trust policies (data protection, equal opportunities, health and safety) Understanding of clinical governance and its implications Understanding of National Health plans and implementations Skills

To work as an autonomous practitioner and have the ability to organise, prioritise and delegate work Proficiency in selecting and using commonly prescribed small aids and equipment (e.g. hoist, standing aids) Ability to demonstrate problem-solving skills Excellent time management and organisational skills Able to present information in a clear and logical manner (written and orally) to keep clear notes Ability to communicate clearly and concisely via written and spoken word Ability to work as an effective team member in the multidisciplinary team Ability to positively influence and motivate others Ability to reflect critically and appraise own performance IT competent Experience of teaching other staff members, patients and other health care professionals Personal attributes
