Detailed job description and main responsibilities To undertake holistic assessment (including those with complex presentations and multi pathologies) making use of specialist clinical reasoning skills. To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting. To actively manage positive relationships with other agencies across the Cambridgeshire and Peterborough Integrated care System involved in the patients pathway. To provide specialist advice to all members of the multi-disciplinary team, service users and appropriate others, of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient. To support with day to day planning and delivery of a high quality care. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving license and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person specification Education/Qualifications Essential criteria Degree or equivalent e.g., BSc, MSc, or Diploma entry level qualification into the Occupational Therapy profession. Current HCPC registration Documented evidence of own continuing professional development Desirable criteria Member of RCOT Fieldwork/Practice Educator qualification Experience Essential criteria Relevant post-graduate clinical experience in a variety of settings In-depth knowledge of holistic assessment, problem-list creation, and goal setting. Experience of equipment provision. Experience of completing moving and handling assessment. Experience of using outcome measures and data to inform on patient outcomes. Desirable criteria Equipment provision Experience working in intermediate care Experience working in admission avoidance and rapid response Exercise Programme Provision Experience of SystmOne Skills & Abilities Essential criteria Time Management and prioritisation skills. Able to demonstrate commitment to high quality care and service provision. Ability to work flexibly as part of a team. Good communication skills.
