Main duties of the job To complete initial and ongoing patient assessments, support patients to identify SMART goals, utilise clinical outcome measures and develop, deliver and progress bespoke treatment plans. To work collaboratively within the team and with external providers to: Enable patients to work towards goals relevant to the in-patient community hospital setting Support the patient and family members/carers with regards to discharge planning Identify further rehabilitation needs to include onward referral to other services as appropriate Support in developing the service in line with national and local strategies Demonstrate critical thinking in the clinical decision-making process. Evaluate the effectiveness of the treatment through autonomous and accountable decisions in Physiotherapy practice supported by current research and in accordance with NHFT protocols and policies. Communicate condition related information, sensitive diagnostic & prognostic information to patients/clients, carers & relatives, requiring empathy & reassurance. Provide leadership, education and support to other team members through promotion of innovative strategies to manage change and improve service delivery.
Minimum Requirements HCPC registration and relevant degree/qualification in Physiotherapy Good communication and organisational skills Manual handling experience Previous healthcare experience
