An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Band 6 Physiotherapist. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating Urgent Community Response service, OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients.

This role will be working for our Urgent Community Response Team which has developed pathways with East of England Ambulance Service and the Frailty Virtual Ward, two new and exciting government initiatives.

Our team delivers excellent patient focused care to patients in Ipswich and the East Suffolk community. Could you be a valued key member of our team?

The role will be for 37.5 hours per week, including weekdays & weekends. The service hours are 8am to 8pm.

Main duties of the job

Provide skilled physiotherapy clinical expertise with knowledge in admission avoidance, frailty and caring for the older person in the community. Manage and escalate patients on the REACT virtual ward (VW) and community caseload (UCR). Promote high standards of professional practice and deliver quality care to aid admission avoidance where appropriate. Plan and provide training/education for nurses, therapists, and other disciplines in relation to admission avoidance. Maintain a high level of awareness of relevant research trends and national recommendations regarding admission avoidance and older person's care/frailty. Be responsible and accountable for ensuring the REACT team complies with clinical governance and quality.

Experience

Essential

Experience of multi-disciplinary working Comprehensive range of junior rotations with experience of complex caseload management. Experience of facilitating discharges from the acute setting and/or managing a caseload of patients within the community. In-service training and reflective learning Ability to work as an autonomous practitioner Prioritisation of workload
