East Bergholt Surgery and Constable Country Medical Practice has a fantastic opportunity for an experienced senior practice nurse to join our team on a full-time /4 compressed days or 5 shorter days basis. This position is to work across both our sites and will be offered on a 12 month fixed term contract.
Main duties of the job
Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long-term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations. Implementing, and participating in, vaccination and immunisation programmes for both adults and children. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running asthma, COPD and diabetes clinics and reviews. About us

Experience
Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS. Qualifications
Current NMC registration with an active PIN. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates. Disclosure and Barring Service Check
