Senior Practitioner - Older People, Mental Health Team Job description 37 hours per week

This is an exciting time to join our Older People Mental Health Team. The team works in a multi-disciplinary environment to develop a team around the person and their family and carers.

We are seeking to appoint a creative, enthusiastic, ambitious full-time, permanent Senior Social Work Practitioner within the Older People Mental Health Team. The successful candidate will support the Team Manager in the day-to-day running of the team, undertake some supervision of other team members, and carry a small caseload of complex cases.

The Team consists of Social Workers, a Senior Practitioner, and a Team Manager. The people we support can have long-term complex mental health challenges as well as needs around their physical health and emotional wellbeing. The team works with older adults and their carers to provide holistic support to allow people to live their lives as they wish to and ensure voice, choice, independence, and control in their lives. The team collaborates with Community Mental Health Nurses, Psychiatrists, Occupational Therapists, and Physiotherapists. We work closely with G.P. surgeries and third-sector partner agencies to assess and support the needs of adults affected by long-term complex and adverse mental health conditions as well as their carers and families.

Using a strength-based, outcome-focused model of Social Work, the team supports adults and their carers to achieve positive outcomes within their lives and encourages wider inclusion in local communities. Maximizing an individual’s emotional wellbeing is seen as key in effective social work practice.

We are keen to continue to develop innovative approaches to social work practice. We work closely with other agencies and partners to prevent unnecessary admissions to hospital and care homes, as part of our role to maximize independence and choice. The continued development of this dynamic service model will require work on widening partnership opportunities with statutory partners, the third sector, and individuals from the community. We are particularly looking for those who are interested in community work and therapeutic social work practice. We are keen to develop resilient community connections, groups, and services in Bridgend. The position will be based in our office in Bridgend, and we will offer focused and committed development opportunities for the successful candidates.

A recognized Social Work qualification and registration with Social Care Wales is essential for this post.

The ability to greet customers through the medium of Welsh is a requirement for this post.
