Job summary Are you looking for a band 6 community job without a caseload? Keen to no longer work shifts and work hours that suit you? This is a 4 days a week role. We are looking for an experienced individual to support service users requiring increased support from the Community Mental Health Team (CMHT). The team has a strong group of care co-ordinators; however, there are times when service users require more input than a care co-ordinator can provide. At these times, we utilise 'shared care'. We are looking for an individual to lead on the 'shared care' work. This is pre-planned work supporting service users who may have increased risk, emotional distress, or relapse in their mental illness. There is an expectation of joint working with the Crisis Resolution Home Treatment and Mental Health Liaison Teams. As part of this role, you may be asked to 'in reach' service users admitted to a ward or to complete 'soon' assessments for service users referred to our service. Along with the shared care role, there will be a need to complete new service user initial assessments to understand care needs and to plan care or signpost to the most appropriate service. As the first point of contact for our team, we are looking for an approachable, professional, and compassionate individual. Main duties of the job To fulfil the requirements of this role, you need to hold a professional registration in Social Work, Mental Health Nursing, or Occupational Therapy and be competent in completing high-quality assessments of individuals' needs. You may be required to cover clinics as necessary (qualification dependent). To be an active member of the multi-disciplinary team supporting the assessment and care planning for service users. To support patients with complex needs who are in a community crisis and require shared care. Care co-ordination for patients with severe and enduring mental health difficulties. To undertake ongoing assessment, treatment, and evaluation for service users known to the service. To support the delivery of therapeutic interventions. Support the transformation of the team to deliver local evidence-based care. About us Hampshire and Isle of Wight Healthcare NHS Foundation Trust is a newly established organisation, bringing together expertise from across mental health, learning disabilities, community, and physical health services. We are dedicated to delivering compassionate, high-quality, and accessible care to the diverse communities of Hampshire and the Isle of Wight. With over 13,000 dedicated staff working across more than 300 sites, we place patients and staff at the centre of everything we do. Our goal is to create an integrated healthcare system that improves accessibility and ensures more consistent care for patients. By working together, we aim to improve patient outcomes, drive innovation, and meet the unique needs of the populations we serve. Join us as we embark on this exciting journey to shape the future of healthcare across Hampshire and the Isle of Wight. We are deeply committed to our CARE values of compassion, accountability, respect, and excellence, and offer outstanding opportunities for career development, training, and collaborative working.
