We are well on our journey to delivering collaboration across Health and Social Care to improve outcomes for those living with frailty in Warrington. You will contribute to a team to support continuous development and operational effectiveness, support and lead in the absence of the Team Manager, and provide formal support and supervision to team members, including professional supervision and holding your own small caseload. As a member of a forward-thinking, supportive, and ‘can do’ team, you will have care management skills, hold a current registration with Social Work England, possess multi-disciplinary work experience, and have excellent interpersonal skills, enabling you to work in genuine collaboration with others, with a strong person-centred value base.
