Senior Respiratory Specialist Practitioner

Respiratory Hospital at Home is designed to facilitate early/supported discharge of patients from acute medical beds and to prevent hospital admissions of patients with unstable respiratory disease in the community, creating a 'virtual ward'. You will provide a high standard of complex, enhanced care for patients, using advanced clinical judgement, skills, and knowledge to avoid admission to hospital or support an early discharge. At times you will be part of an exciting pilot working within a Care Co-ordination hub, collaborating with system partners including SWASFT, 111, Rapid Response, Somerset Primary Link, and District nursing teams to ensure patients are seen by the right team, in the right place, the first time. Main Duties of the Job

Senior Respiratory Specialist Practitioners work as part of a multi-disciplinary clinical team across various settings, including hospitals, individual homes, care homes, and general practices. You will work at an enhanced level of practice with specific knowledge and skills in respiratory medicine. You will manage a discrete aspect of a patient's care within your current level of practice, specific to a particular context. You will provide high standards of complex, enhanced care for patients, using advanced clinical judgement, skills, and knowledge. We are looking for staff who can support patients with respiratory illness in their home environment. You will work alongside the Clinical Leads to lead and manage a team of highly skilled practitioners. You will co-ordinate and manage shifts, ensuring patient and staff safety is prioritized, ensuring referrals to the service are clinically appropriate and safe. You will work alongside the care co-ordination hub to support patients in receiving the right care, in the right place, by the right team, the first time. You will lead and manage members of the clinical team, ensuring staff are working to their clinical competence with areas of progression highlighted, challenging any behaviours or practices not aligned with the Trust Values. About Us

Respiratory Hospital @ Home is a county-wide service providing care for patients who have a Somerset GP. We have four community bases located at Harrison House, Bridgwater Hospital, West Mendip Hospital, and Magnolia House, while supporting the two acute sites of Musgrove Park Hospital (MPH) and Yeovil District Hospital. This post is being advertised to cover the West side of the county, which includes the Taunton/Bridgwater area alongside MPH. We do cross cover within the service, and you will be expected to support the South side of the county when patient needs arise, which would be on a weekly basis. Job Responsibilities

Qualifications

Registered Nursing degree or equivalent in physiotherapy Registered with the Nursing Council or Physiotherapist equivalent Relevant postgraduate study in leadership or clinical study related to clinical care Teaching qualification or equivalent experience Non-medical prescribing, or willingness to work towards Experience

Experience of managing respiratory patients within a community and acute setting Understanding of working as part of a MDT Enhanced understanding of respiratory conditions Additional Criteria

Proven leadership and management experience with respiratory patients Proven evidence of educational support to others Experience in a leadership/management role Evidence of research and audit participation and an understanding of the process involved Experience of developing guidelines, protocols, and pathways of care Good understanding of the NHS framework and government policy Familiar with current local and national issues Evidence of a good standard of Literacy/English language skills Able to demonstrate enhanced decision-making/clinical reasoning skills through analysis and interpretation Ability to take overall responsibility and accountability Self-motivated and able to work under their own initiative Problem-solving approach Innovative and creative, able to think laterally Able to influence and motivate other staff Team player with a 'can-do' attitude Personal resilience Drive, energy, and enthusiasm Always act in a professional manner demonstrating honesty and integrity Adaptable and flexible to the needs of others and the organization Demonstrates punctuality and good timekeeping Develop and maintain good working relationships within the Neighbourhood, Acute setting, and with Stakeholders Willingness to use technology to improve standards of care and support to our patients Ability to cope with potentially distressing situations such as imparting unwelcome news Strong prioritization skills with the ability to manage competing demands Physically able to do the work described Concentrate for long periods on a daily basis Complete work whilst being subjected to frequent unpredictable interruptions Access to a vehicle for travelling within and outside the trust boundaries Disclosure and Barring Service Check
