The Integrated Care Homes Service is looking for Band 6 Senior Staff Nurses to join the service. For registered nurses (two years post graduation along with community nursing experience) who are looking to further their career and develop new skills in leadership. This vacancy is a permanent position working 27 hours a week. As a Registered Nurse, you will provide nursing interventions to a wide range of patients, by in reaching into Nottingham City Care Homes to deliver NHS services. The service's overarching aim is to ensure that care home residents and staff have an improved experience through high-quality care and access to familiar health professionals with the skills to meet their needs. This includes providing support to the staff within a care home setting in developing a plan of care for the patient. The successful candidates will be required to have up-to-date clinical knowledge and skills and will be required to work autonomously, using initiative to deliver nursing care within a supportive team environment. They will be working with a diverse population providing support and nursing care to people who have complex health problems, with the aim to maintain and maximise their independence and prevent avoidable hospital admissions. The role of the Senior Staff Nurse is to provide nursing interventions to patients whose home is within a residential care home. This includes insulin administration, wound care, continence care, palliative support, and EOL care. As a SSN within this integrated service, you will be working closely with colleagues from dementia outreach and physical therapy facilitating your development of new skills and knowledge across all the elements of the service to enable a holistic approach to patient care, improving patient outcomes. The SSNs will work across Nottingham City supporting the 3 District Nurse Team Leaders with their caseload management and staff supervisions, while providing a high level of clinical support and guidance to junior staff and students. The service plays a key role in implementing the Enhanced Care in Care Homes Framework by providing support to care home staff, enabling them to deliver optimal care to their residents. This includes supporting staff within the care home setting to upskill and empower them to deliver optimal care to their residents, as well as developing communication pathways within care homes to enable proactive care and prevent avoidable hospital admissions. Job Purpose To have continuing responsibility for the maintenance of a high standard of care to the patients and carers on the DN caseload. To contribute to the continuing responsibility/management of the caseload, DN team, and related resources. To contribute to strategies and policies for DN service improvement and development. To take an active role in the delivery of the Public Health agenda. Dimensions To be cost conscious. The case load size will be variable. To provide mentorship and leadership to clinical staff working within the specialty. To work collaboratively and across organizational boundaries to provide evidence-based practice and holistic care to patients in a variety of settings. Key Responsibilities To be responsible for the holistic assessment of the health and wellbeing needs of referred patients and carers, using analytical and interpretive techniques. Contribute to the Health Equity Needs Assessment of the local community. (Community Health Profile) Contribute to the Public Health agenda by raising awareness of health and wellbeing in the local community. To have continuing responsibility supporting the District Nurse Team Leader with the planning, delivery, and evaluation of programmes of care to address the complex and changing health and wellbeing needs of individuals and communities. Utilise enhanced skills and knowledge to provide specialist technical services and advice in relation to care e.g. palliative care, leg ulcers, continence. Implement the provision of evidence-based/best practice. Participate in public health and health promotion activities in response to identified local need. Contribute to the involvement of the public in the planning and evaluation of the District Nursing Service. Actively encourage patient and carer feedback about the DN service. Act as a role model, setting, monitoring, and evaluating the high standards and continual improvement to the DN service. Maintain a professional image as outlined in the NMC Code of Conduct. Promote the Clinical Governance agenda and apply risk management and reporting systems. Participate in relevant research and audit activity. Promote the provision of care by the team, which is consistent with evidence-based/best practice. Identify needs and prioritise work appropriately to manage the delivery of the service. Take responsibility for the appropriate delegation and supervision of work according to the skills and abilities of team members. To be responsible for ensuring that the standard of record keeping by self and the team meets PCT and Professional standards. Establish and maintain communication with individuals and groups in a wide variety of settings and situations at all levels within the organisation and the wider health and social care community. Establish and maintain effective team working within the DN and Primary Health Care Team. Motivate the team and implement change management processes. Set team objectives to continuously improve the standard of care and develop the service. To have continuing responsibility for performance monitoring of team members e.g. sickness and absence, competencies, complaints. Regularly has responsibility for the management and coordination of the PCT wide DN service at weekends and bank holidays. Contribute to the recruitment of staff through involvement with short-listing and interviews. To be responsible for the development of own and team’s knowledge and clinical practice. To be responsible for the performance review and development interviews of the team in order to identify training and development needs. Participate in the supervision framework both as a supervisor and supervisee. Mentor and assess a variety of students including nursing and allied professionals. To be actively involved in the continuous development of the DN service and policies, procedures, and guidelines. Represent the PCT DN service on multi-professional developments which support interprofessional working. To be responsible for the promotion of best practice that ensures the health, safety, and security of self, patients, and colleagues in accordance with the policies, procedures, and guidelines of the PCT. Special Criteria

Car driver in possession of a full, current driving licence (and access to own vehicle). Able to work on a rota basis. Knowledge and skills

Up to date clinical knowledge and competence in wound care, management of continence, palliative care, diabetes. Current knowledge of Local, Regional, and National policies. Evidence of continuous professional development relevant to the role. Effective advanced communication skills, verbal and non-verbal, and experience of teaching. Able to record accurately, clearly, and concisely. Ability to work within a culturally diverse population. Ability to work as an autonomous practitioner. Evidence of working with external agencies and networking skills. Ability to work autonomously, within a variety of settings, managing own workload and changing priorities. Flexibility to fit in with the needs of the service. To work as part of a team. Experience

Post registration experience within community nursing. Experience of caseload management. Evidence of good interpersonal and leadership skills, and mentoring staff and students. Evidence of working in a multidisciplinary team. Qualifications

RN qualification on part 1 or part 2 of the register. 2 years community experience. Post registration qualifications relevant to community nursing including training. Disclosure and Barring Service Check
