Working in partnership with the criminal justice system and other agencies, we provide responsive, dependable and high-quality primary care and mental health services to existing offenders (and those at risk of offending) in the communities we serve, to improve their health status and to encourage social inclusion.

Main duties of the job

The post holder will deliver an expert healthcare service to patients. They will offer advice to clinicians, allied healthcare professionals and the patients.

To provide a comprehensive primary care physical health service and work primarily within primary care which is part of an integrated healthcare service which includes Primary care, Substance misuse and Mental Health.

To undertake physical health assessments, undertake clinics, provide and evaluate a range of health treatments and interventions, to meet the healthcare needs and contribute to the wider continuous development of healthcare services.

Demonstrate knowledge, skills and understanding of health issues and treatment options to provide physical health clinical guidance to all staff on matters relating to physical health and wellbeing.

To ensure high standards of primary care services are delivered in a changing healthcare environment, ensuring that the needs of the women are at the centre of practice.

To ensure that the Healthcare Team is integrated within the centre and that there is excellent communication and working relationships with all stakeholders, both internal and external.

Is expected to continually promote high standards of service within the nursing team and contribute to development of a culture of professionalism within the team.

To participate in a flexible working arrangement which will include weekends and unsocial hours when required.

Job responsibilities

To plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence based models To set up processes/protocols for effective assessment, planning, implementation and evaluation of care for those patients with physical and mental health and substance misuse To safeguard vulnerable patients in line with Trust safeguarding To apply local Risk Assessment and management To make appropriate referral to other available services for longer term Co-ordinate multi-disciplinary clinical practice to provide focused and integrated care and treatment programmes/packages. To be aware of the appropriate handling of aggressive patients and the appropriate procedures for dealing with Work collaboratively with staff of other disciplines e.g. supervisors and directors to provide integrated and coordinated care To work within own competency and ensure that any training needs are communicated to the team To participate in local audits and trust audits to enhance quality of care. To participate in clinical governance and utilise research skills for audit, service development and research within the area served by the multi-disciplinary To liaise with other professional services (hospital staff, GPs, caseworkers, sharing information where necessary) To work in a highly emotive atmosphere with clients with offending histories and / or mental health problems on a regular basis, and be able to communicate sensitive and complex information, whilst maintaining a high degree of professionalism at all times. To attend and contribute to centre meetings as required, and to present the view of Healthcare Team for relevant clinical issues. To ensure any actions taken from these meetings are met or that the appropriate individual to carry out the action is identified.

Person Specification

Education / Qualification

Essential

Registered General Nurse or Registered Paramedic with substantial post qualification experience

Desirable

Mentoring (e.g. ENB 998 or MSPP)

Knowledge

Essential

Understanding of clinical governance issues in primary care and secure settings Knowledge of PGDs and associated policies

Understanding of health promotion and its delivery
