An exciting opportunity has arisen to appoint a Service Development, Integration, Transformation and Delivery Lead within the Age Well portfolio on a permanent basis within NCL ICB’s Strategy and Population Health Directorate for the borough of Camden or Islington. We are looking to appoint two individuals to lead the core offer for community services for Camden and Islington. This role is ideal for individuals who have a strong passion to improve population health for NCL’s 1.7m residents by delivering a more proactive model of care closer to people’s homes. Main duties of the job
This role delivers both borough-specific and NCL-wide capacity to drive specific priorities relating to service development with Service Development staff embedded within the BIUs (Borough Integration Units). You will play a key role in shaping service development delivery through place and will need to collaborate with colleagues within Place Development to co-create the NCL ICB approach to delivery locally, in line with our population health strategy. The post holder will work with peers across Service Development to implement change NCL-wide, focusing on improving health system productivity, ensuring financial sustainability for the NHS, and driving improvements in health equity. This role will be line managed by the Service Development: Integration, Transformation and Delivery Senior Manager (Age Well) and will closely work with the Assistant Director for Community Service Development. Education / Qualifications
Educated to degree level, or equivalent level of experience in this area. Excellent knowledge of project/programme management principles, service delivery, business processes, and office systems. Experience
Proven ability to deliver a portfolio of work on time and within budget, monitoring and reporting on risks and outcomes. Demonstrated experience of relationship building and partnership working with a range of internal/external stakeholders. Knowledge/Skills/Abilities
Able to act upon incomplete information, using experience to advise and support, make inferences, problem solve, and make decisions. Advanced Microsoft Office skills, and ability to learn other packages or databases as required. Awareness of the 'golden thread' from the ICB's Population Health & Integrated Care Strategy to this role; supporting and delivering better outcomes for the patients/residents of NCL.
