Main duties of the job This is your chance to be part of a vibrant team dedicated to providing exceptional care to patients in the comfort of their own environments. As a pivotal member of our team, you'll not only deliver high-quality nursing care but also mentor and support junior staff, fostering growth and collaboration every step of the way. In this role, your autonomy will shine as you assess and attend to the diverse health needs of our community, including those facing end-of-life challenges and patients with long-term or complex conditions. Working under the guidance of the Integrated Team Leader, you'll leverage your clinical expertise to make vital decisions that directly impact patient outcomes. Your days will be dynamic and fulfilling, blending hands-on patient care with the gratifying task of nurturing emerging nursing talent. Qualifications
Up to date professional portfolio demonstrating evidence of reflective practice
Ability to travel to different geographical locations across the Trust footprint in line with service needs
Full UK driving license with willingness to use own car for work purposes.
Knowledge, Skills, Training and Experience
Demonstrate excellent communication skills both written and verbal
Ability to plan and organise own work
Aptitude to work cohesively within a team
Computer literacy skills
Awareness of community working and care pathways
V150 prescribing.
Good interpersonal skills
Leadership potential
Management of change
Relevant Community experience
