Sister/Charge Nurse – Critical Care Outreach

We are currently recruiting for permanent members of our team - we have vacancies for one full-time position and one part-time (0.69 WTE equivalent to 9 long days/nights).

As a Critical Care Outreach Nurse, you will play a pivotal role in extending critical care expertise to non-intensive care areas, ensuring early identification and management of deteriorating patients. This position demands a strong commitment to patient safety, effective communication, and collaboration with multidisciplinary teams. The critical care outreach team (iMobile) will act as a support for ward based staff and hospital at night team in caring for deteriorating patients.

Services provided by the team will enhance the care of the critically ill patient in delivering specified critical care therapy on the ward with the potential to avoid ICU admission. Where admission to critical care is required, the team will support the patient during the transition from the ward to critical care.

The critical care outreach nurse will also support patients following critical illness, after their transition to general wards. You will also be expected to assist the lead nurse with continuous service improvement for the critical care outreach team and trust-wide initiatives to improve deteriorating patient care, such as deteriorating patient education.

Main duties of the job To work in close co-operation with multi-disciplinary teams, facilitating high standards of health to patients who are critically ill outside the ICU setting. Assess the health needs of the deteriorating patient through physical examination, history taking and diagnostic studies in consultation with the iMobile consultant, senior registrars or ACPs. Develop and manage a plan of care for deteriorating patients, working closely with the MDT, iMobile consultant, senior registrars and ACPs. Work closely with the Hospital at Night team (H@N), medical, nursing, and AHP on the wards. Provide management and leadership support for the iMobile critical care outreach team. Assist the iMobile nursing lead to achieve key performance indicators and success measures for the iMobile service. Actively be involved in audits, research and service improvement projects as part of continuous improvement. Assist in the training of ward nurses in theoretical knowledge, practical skills and in expanding their roles. Assist with the delivery of deteriorating patient education at the trust. To work closely with King's Health Partners and Kings Commercial in the absence of the iMobile nursing lead.

Demonstrates an advanced knowledge in recognising, assessing, and treating critically ill patients. Facilitates teaching both informally and formally as part of proactive critical care outreach.

Person Specification Education, skills and previous experience Essential Intensive care nursing course ALS Provider Significant critical care experience Teaching, mentorship and/or student supervision assessment course Evidence of Continuous Professional Development Degree or near completion Registered adult nurse with current NMC registration Advanced assessment skills through formal training or significant clinical experience. Competent in Microsoft office Evidence of management and leadership skills Experience in completing service improvement projects in healthcare

Desirable Previous experience in critical care outreach Independent prescribing Advanced Physical Assessment skills course MSc in Nursing, critical care, advanced practice or other relevant subject. ALS instructor Quality improvement training
