Enhanced DBS disclosure required. Skills & Experience: NVQ Level 3 Social Care or equivalent. Knowledge and understanding of The Care Act and Safeguarding Adults procedures. Knowledge and understanding of the Mental Capacity Act. Experience of working with service users, carers and other professionals in a health or social care related field. Experience in responsibility for the arranging or managing of care services. Experience of assessing and addressing risk. Excellent recording skills using electronic data information systems, including experience of entering and extracting information from a variety of sources. Effective assessment, planning and reviewing skills, with the ability to prioritise tasks and manage own workload, seeking guidance appropriately. Desirable: Knowledge of resources; health, local authority, voluntary and independent sector. Experience of using a strengths-based approach in practice. What's involved with this role:

You will be providing additional capacity to review the care and support needs of people using the bed based services. You will also provide community discharge support, ensuring timely assessment and transition into any assessed ongoing social care support. Key Responsibilities: Assist Social Work Teams with case work ensuring that families or individuals receive support that is consistent, integrated and is of the highest standard. Provide information for reports, meetings, conferences, etc., to agreed standards. Undertake basic/initial assessments keeping cases under review and maintaining high quality standards of service to Vulnerable Adults. Maintain high quality safeguarding standards for Vulnerable Adults, reporting concerns promptly to the Team Manager. Update and maintain computer and written records in accordance with Directorate guidelines ensuring the quality of the information noted is accurate and up to date. Participate in and/or minute meetings in respect of service users. Establish and promote effective working relationships with external partners, agencies and other sections of the Directorate that will improve practice and outcomes for individuals and families.
