Want to take the next step in your career? We are looking for motivated, experienced professionals and people embarking on a new career, or returning to work, to join us who are committed to working in integrated multi-agency teams, ready to embrace change and new ways of working. As a Social Care Discharge Coordinator, you will work in partnership with the Integrated Discharge Team to manage the discharge pathway for a population of clients moving through the acute hospital setting, working closely alongside other agencies and professionals. You will be responsible for triaging and allocating referrals to appropriate workers in order to facilitate safe and timely discharges for people who are on a hospital discharge pathway. You will supervise and provide complex case management advice to a team of Registered Practitioner Case Officers. You will also be responsible for accurate data collection and escalating any issues affecting the functionality of the team to the Senior Practitioner and Team Manager. The Integrated Discharge Team operates a discharge to assess model of care which involves patients having their assessments outside of the acute hospital environment, and you will be supporting clients as they move through and beyond this pathway. Adult Social Care and Health operates within modernised office processes and performance management systems. The service is increasingly accessible to the public outside of traditional office hours, as we move towards an 8am to 8pm, 7 days a week service. OPPD staff and managers are increasingly working on a rota basis to ensure service needs are met. Alongside opportunities for training and development, you will receive the support you need and recognition for your good work. This is an exciting time to join us, and informal visits prior to interview are welcomed from people who share our values and are ready to be part of our exciting journey. Minimum Requirements: Registered as a social worker with Social Work England. The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. This post is subject to an Enhanced Disclosure application to the Disclosure and Barring Service and registration as a qualified Social Worker with Social Work England. This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. If you would like to discuss this opportunity further, please contact: Work that works for you and us - let’s talk flexibility! KCC is committed to building a workforce which reflects the diversity of the county’s working residents, encouraging applications from people of all ages, abilities, genders, sexual orientations, ethnic backgrounds, faiths, and those with caring responsibilities, and aspires to create an inclusive workplace where everyone can be themselves at work. Please note – if you are interested, please apply as soon as possible as the closing date for this advert may be earlier than stated should a number of suitably qualified candidates apply.
