An opportunity has arisen for a Full-time Social Care Practitioner to join South Derbyshire North Adult Social Work Team. We are a supportive and person-centred team supporting young adults in transition and adults over 18 years of age. The successful candidate will be based in South Derbyshire with hybrid working. Training and support will be provided, which includes regular supervision, peer support, team and area meetings alongside access to Derbyshire County Council training. Key Responsibilities

Work in partnership with clients, their families, and carers to undertake Care Act (2014) assessments. Create support plans and conduct regular reviews using person-centred and strength-based approaches. Commission support and signpost services promoting the home-first culture. Support residents with safeguarding concerns and work in accordance with The Mental Capacity Act (2005). Collaborate with stakeholders such as care homes, housing departments, and NHS to provide a multi-disciplinary approach. Travel using your own transport for which a mileage allowance is claimable.
