We are looking for enthusiastic and motivated individuals to join the existing team, extending the current personalised care to a wider patient population. The successful candidates will be able to demonstrate the ability to be a good listener, have time for people, be committed to supporting local communities to care for each other and reduce health inequalities. You should have experience of working positively with people facing complex social and emotional challenges in either a paid or voluntary capacity; inspiring trust, building confidence and motivating. You will have great interpersonal skills; be empathetic, resilient and be able to maintain effective working relationships with partners across the sectors.
