If so, Randstad is currently seeking a Qualified Social Worker to join the Hospital Discharge team at Fairfield General & North Manchester General Hospital in Bury. This full-time position offers an exceptional pay rate of £30 per hour.

Responsibilities:

  • Assist residents with discharge arrangements, ensuring timely and safe transitions to suitable settings.
  • Implement various Discharge to Assess and re-ablement pathways to support individuals in maximising their independence outside of hospital settings, emphasising discharge to appropriate locations at the right times.
  • Work collaboratively with individuals to assess immediate and ongoing needs, facilitating the implementation of long-term care and support plans.
  • Facilitate discharge planning across multiple hospital sites within and beyond the borough, as part of a multi-agency team, to ensure effective, safe, and timely discharges.
  • Conduct Mental Capacity assessments, Best Interest Meetings, and attend Multi-Disciplinary Team (MDT) meetings as required.
  • Participate in a duty rota.

Requirements:

  • Registered with Social Work England
  • Enhanced DBS clearance
  • Bachelor's degree in Social Work or equivalent

Experience:

  • Specialised experience working with adults
