Integrated Discharge Team (Hospital) Salary:

£38,364 to £48,060 per annum Contract:

Permanent Purpose of the role: Use skills, knowledge, and understanding of practice within the area of specialism to support and deliver in the development of service provision. Make appropriate use of local, departmental, and independent resources. Maintain high standards of professional practice within your own workload. Use advanced skills, knowledge, and understanding of the area of specialism to assist in the development of service provision and promote independence, choice, and control for service users. Carry out all duties within a quality and equalities framework based on Council policy and performance targets, professional standards, and best practice. Main duties: Practice supervision for students on placements. Provide support to Care Coordinators as required. Ensure high-quality standards are upheld throughout all aspects of assessment and service delivery. Implement the Safeguarding Adults policy & procedure. Adhere to the Council’s Health and Safety policies. Keep abreast of developments in all relevant areas concerning professional practice, relevant legislation, and Central Government guidance. Adhere to HR policies and procedures. Manage, monitor, and review care plans involving users and carers, liaising with other workers and agencies as appropriate. Requirements of the Social Worker: Degree or equivalent in social work. Current Social Work England registration. Significant frontline experience in Adults Services. Ability to implement a self-directed approach in line with the personalisation agenda and the Safeguarding Adults policy for the Council. Ability to communicate clearly and effectively both orally and in writing, including report writing, presenting, negotiating, and the use of IT. Experience of delivering services using computerised systems. Experience of providing services to vulnerable adults. Experience of performance management. Awareness of the Health and Social Care integration agenda. Contact:
