What the role involves

• Undertake strengths-based Care Act assessments.
• Provide information, advice and signposting to residents and carers.
• Manage demand within an Access/Duty setting.
• Develop person-centred support plans focused on outcomes.
• Undertake and contribute to safeguarding enquiries.
• Manage risk, including applying Mental Capacity Act and DoLS principles.

Skills and requirements

• Qualified Social Worker (Degree/DipSW/CQSW).
• Experience within Adult Social Care , ideally in Access, Duty or Front Door services.
• Strong understanding of the Care Act 2014, Mental Capacity Act and safeguarding frameworks.
• Ability to manage a busy caseload and work under pressure.
