Responsibilities:

Hold a client case load, carrying out Care Act (2014) assessments and care planning to meet assessed eligible needs.

Conduct assessments in hospital or reassessments following hospital discharge to support the Integrated Community Service purpose.

Carry out Mental Capacity Assessments and Best Interest decisions.

Participate in multi-agency meetings, working with partner organisations, colleagues, the person, and families to provide effective outcomes that meet the assessed needs of residents of Shropshire.

Complete CHC checklists and participate in Decision Support tool meetings.
