Social Workers Triage and Response Team Bristol City Council £32 per hour Umbrella company Bristol City Council is looking to recruit 3 social workers to join their Triage and Response Team, which is integrated within the discharge teams at Southmead Hospital and Bristol Royal Infirmary Hospital. Initial contracts offered until mid March 2025. Full-time hours required. There is a requirement for social workers to be in the office every day unless there are reasonable adjustments that BCC will need to consider. To be considered for this role, you will have a passion for enabling service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support. Responsibilities include: Completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation. Developing comprehensive discharge plans. Collaborating with medical staff to ensure seamless patient care transitions.
