An exciting job role has recently become available at the Vale of Glamorgan Council. They are looking for a new member for their Multi-Disciplinary Team. The team offers specialist secondary mental health care for adults living with dementia and people over 65 years with mental health problems. The team works alongside specialist NHS services to support people to live well and safely in their own communities through the provision of recovery-focused care and treatment. Job Duties: To undertake assessment of citizens in accordance with statutory duties under relevant legislation. To plan and commission services to enable adults with care & support needs and carers with support needs to overcome barriers to achieving personal outcomes and monitor and review care plans through co-production with citizens and carers. To work in partnership with citizens with care and support needs, carers with support needs, and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them. To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers. To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately. To maintain up-to-date electronic case files and relevant documents, providing accurate records relating to performance management and other statistical data as required. Requirements: Social Care Wales Registration. Experience of working with adults with long-term conditions. Experienced Qualified Social Worker Required. Experience of safeguarding. Experience of working with adults and self-neglect. What we are looking for in a Social Worker: Social Work England registration. Eligible to work in the UK. Hold a full UK driving licence.
