This post is based at the Western Isles Hospital in Stornoway, Isle of Lewis and offers an excellent opportunity for an experienced self-motivated pharmacist to help continue exciting work in developing a clinical and patient-focused clinical pharmacy service. You will get the opportunity to make a real difference to the pharmacy service, pursuing value-added projects which resonate with you.
The post-holder will be one of three clinical pharmacists working at this hospital and will be responsible for the clinical pharmacy service to a number of wards including consultant ward rounds, patient counselling, and multi-disciplinary working with ward-based medical and nursing staff. There is a technician-led service to the wards to provide support with medicines reconciliation and counselling.
There will also be opportunities to get involved in providing a clinical pharmacy service to the local hospice and remotely to the other two hospitals in the archipelago, SACT, and multi-disciplinary training sessions. All hospitals in the Health Board are utilising HePMA. Development of managerial and clinical skills will be encouraged through project work and audit.
This is an exciting opportunity to join a friendly, supportive, and highly motivated multidisciplinary team including a full range of healthcare professionals. We have close links with the primary care team and work cohesively to put medicines-related projects in place across the patient population.
