As a member of the multidisciplinary Memory Assessment Service (MAS) team, you will function as an independent specialist practitioner, playing a key role in the effective assessment, planning, consultation, and formulation processes toward accurate diagnosis.

Key responsibilities include:

* Utilizing specialist knowledge and expertise in strategies and interventions for the treatment of dementia.

* Efficiently referring patients to appropriate internal services or external agencies when necessary.

* Identifying and managing risk factors with confidence and taking appropriate actions.

* Facilitating communication between GPs, services within LCFT, and non-statutory organizations to ensure the highest standards of patient care.

* Providing mentoring and clinical supervision to staff members and trainees on placement.

* Actively promoting the Trust's values within the MAS team and adhering to the professional standards outlined by the NMC and HCPC codes of conduct.

* Assisting the team manager in ensuring effective leadership within the MAS team, contributing to the assessment, formulation, planning, and monitoring of care provided to patients and their carers.

This role is integral to delivering high-quality care and support for individuals with dementia and their families, fostering collaboration and excellence within the MAS team
