Are you ready for a brand new development opportunity? We are looking to recruit a highly skilled Mental Health Practitioner (OPMH RMN/RGN) to join us as a key member of our multi-disciplinary team working closely with Urgent Community Response to ensure that the physical and mental health needs of older adults are addressed. You will provide urgent response to older adults who are experiencing a health crisis and are living in care homes. This is a new post to reduce avoidable hospital admissions, minimise placement breakdown and reduce the use of anti-psychotic treatments. A secondment into this role would be considered for the right person. Main Duties of the Job

To provide a rapid response to older adults who are experiencing a health crisis who are living in care homes and provide specialist mental health care assessment and intervention. Patients may present with organic or functional mental health conditions, complex psychosocial and behavioural difficulties. To lead the service to provide a psychologically informed service where a behavioural approach to care provision is embedded and communication is provided to the patient, carers, care home staff, families and other service providers supporting the approach. To provide assessment, formulation, diagnosis (for appropriately trained clinicians) and interventions -- which may include the prescribing of medications. To work positively and collaboratively as an integral part of the Older Peoples Mental Health services and effectively liaise with multi-agency colleagues, patients and their families. To provide clinical oversight, monitoring and effective caseload management processes for the team covering the geography. To support delivery and oversight of the OPMH and UCR team's key performance indicators. To provide mental health education, advice and training to clinical staff and students -- ensuring the provision of education facilitated by the EHCH Quality Team. To provide education, group or 1:1 clinical supervision and/or reflective practice of band 5 and 6 clinicians and any other staff member deemed appropriate to receive supervision from this post holder. Qualifications

Appropriate relevant professional qualification, Mental Health Nurse, SW, or OT Evidence of other development and/or training which indicates a commitment to continuous development and preparation for management Willingness to undertake Nurse-prescriber qualification or pharmacology training Masters or Additional post-graduate qualification in dementia or related to mental health care. Quality improvement, management, research or leadership qualification and/or training Nurse-prescriber qualification or pharmacology training. Psychological therapy qualification or training. Experience

In depth experience in the effective and positive management of complex and high risk cases Ability to demonstrate a commitment to multi-disciplinary team working Demonstrate the ability to work collaboratively with other agencies Able to illustrate examples of service development initiated by the candidate Willingness to demonstrate the potential for 'change management' approaches which extend existing practices Exposure to, understanding of and competence in IT facilities and systems associated with service users, budgets and management information Evidence of experience as a supervisor, including a clear understanding and application of an effective caseload management system Demonstrate planning skills, e.g. in terms of resource allocation and funding Demonstrate an understanding of, and a commitment to the principles of recovery, choice and personalisation Commitment to attend staff training and participate in own development through supervision and performance development. Previous Experience of Liaison Psychiatry work, in a service which spans the adult and or older age group. Experience of working in inpatient or community OPMH teams at Band 6 level. Experience in acute care (crisis/AMHT) at Band 6 level. Additional Criteria

Must have access to a car with full UK driving licence Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. Salary: £46,148 to £52,809 a year based on full-time hours.
