We are currently looking to recruit a motivated and experienced Band 6 Mental Health (MH) Practitioner to join our Wisbech Neighbourhood Team (NT) Older People's Service. This is a great opportunity to join our effective and friendly team working with a client group with more complex mental health needs.

You will not only be part of an integrated team of mental Health practitioners including CPNs, psychologists, Consultant Psychiatrist, Support Worker, Social Workers, but will be collocated with district nurses and therapy staff, enabling effective coordinated care delivery.

We provide service-user focused, evidence-based interventions for people aged 65 and over living in their own homes/care homes with either functional or organic mental health illness. We provide high quality assessment, treatment, and care coordination which is informed by your professional bodies and National policy.

The post has opportunities to develop skills and knowledge via training, working alongside experienced practitioners and to be involved in service improvement.

Main duties of the job

You will have the opportunity to manage a caseload of service users, delivering a diverse range of clinical skills delivering therapeutic and recovery-based interventions. We seek to empower service users and carers to actively engage in their recovery journey, working alongside a range of third sector organisations.

The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure at times, and with a desire for innovation. We support agile working for staff, using all appropriate I.T equipment and support.

This service covers Wisbech and the surrounding rural areas.

Job responsibilities

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions. Offer advice and guidance to colleagues in the neighbourhood team on common mental health conditions. Participate in MDT meetings and ensure key information is recorded in shared care plans. Work closely with the mental health integrated care team in order to provide an excellent mental health service. Clinical Assessment of mental health needs.

Person Specification

Education / Qualifications

Essential

Relevant professional registered qualification (Registered Mental Health Nurse or Occupational Therapist or Social worker). Training in supervision and appraisal.

Desirable

Post qualifying training. Non-medical prescribing qualification. Pertinent qualifications to the specialism.

Experience

Essential

Significant relevant experience in working within a mental health service. Registered Student mentor.

Desirable

Experience of working with an older people's community mental health service. Have lived experience of mental health challenges. Experience of clinical and managerial supervision of junior staff.

Skills & Abilities

Essential

Able to demonstrate commitment to high quality care and service provision. Excellent clinical skill in assessment and interventions for common mental health conditions. Excellent communication skills and ability to summarise information and communicate with colleagues. Excellent understanding of the needs of carers. Ability to communicate effectively with people in distress or with communication difficulties.

Desirable

Experience as safeguarding investigator. Experience of doing Best Interest Assessments. Experience of working with nursing home and care providers.

Knowledge & Understanding

Essential

Understanding of statutory framework and care commissioning including: safeguarding, commissioning of care and Mental Health Act and Mental Capacity act. Knowledge of research in field.

Physical Requirements

Essential

Ability to travel in the community. Ability to sit for sustained periods of time.

Other

Essential

Positive approach to older people. Willingness to embrace integrated model and new ways of working. Recognise peoples right to privacy and dignity, treating every person with respect. Willingness to be flexible in approach and attitude.
