We are pleased to advertise the vacancy within our Heart Failure Team for an experienced heart failure specialist nurse band 7, or we can offer a developmental band 6 role with progression to band 7. It is a permanent position 30 hours a week.

Our team provides specialist care for heart failure patients during an acute admission, and in the community. We are passionate about the care we provide to our patients and carers and also in supporting each other.

Job responsibilities

The role will be predominately based in the Community with opportunity for inpatient work too. The post holder will manage a caseload via clinics, home visits and phone appointments. They will support the patient through their journey, providing self-management guidance, symptom management, medication treatments, psychological support and palliative care. Our Heart Failure Clinical Nurse Specialist service also delivers a rapid access diagnostic clinic and virtual ward.
