As an occupational therapist working within the Adult Community Therapy team, you will have the opportunity to champion the key values of your profession.

We understand through purposeful & meaningful activity we can support people to rehabilitate, recover and self-care working alongside a knowledgeable & supportive team of therapists. We support one another to meet the needs of patients living in Lincoln, Gainsborough and the villages in between.

Working beyond boundaries is the only way to support patients in their goals. We encourage creativity, inclusivity & thinking outside of the box.

Your caseload will be varied: you will have the opportunity to work alongside adults who are experiencing a range of long-term health conditions and acute deteriorations in their functional abilities. A key aspect of this will involve assessment of frailty and providing meaningful support for people with palliative health conditions.

We work with the wider multidisciplinary and multiagency teams to ensure that our patients receive high quality and equitable care.

To assess and set goals for patients living in their own homes and care settings To provide high quality occupational therapy interventions To provide clinical leadership and supervision to members of the therapy team To represent Adult Community Therapy in the context of the wider multidisciplinary and multiagency team To work alongside the therapy leads in order to innovate and develop the therapy service To demonstrate a commitment to personal development.

Essential

Degree or equivalent diploma in Occupational Therapy Current Health and Care Professions Council (HCPC) Registration Post graduate experience as an occupational therapist in rehabilitation with older people and/or long term conditions and/or community/community hospitals/in-reach Evidence of leadership experience

Desirable

Member of Royal College of Occupational Therapy Clinical Educators qualification Experience of working in community and community hospital settings
