We are looking for an enthusiastic Occupational Therapist to join our dynamic and supportive Frailty Team. We are looking for an Occupational Therapist who is comfortable completing holistic assessments but who can also provide specialist intervention in long-term conditions, cognition & capacity, and moving & handling.

As a frailty team, we can support front door frailty and start patient assessment from their initial admission. In addition to working with the front door frailty team, we have a short stay ward where we aim to assess and discharge patients within 72 hours if possible. This is to reduce the potential for hospital-acquired functional decline or deconditioning.

If a patient's needs are more complex and a longer period of assessment is required, we have the ability to transfer patients to a ward where this is possible to achieve.

Throughout this process, we work closely with MDT colleagues to ensure thorough assessment and open communication. Our service is innovative and forward-thinking with a commitment to quality improvement and service development. We are fully committed to the training, education, and development of our staff.

We can support applicants returning to practice and will consider developmental band 5 to band 6 applications.

Main duties of the job

As a key member of the Occupational Therapy Team, you will be involved in further developing this role within a Frailty specialism. We have a state-of-the-art frailty block that has been purpose-built for the frailty pathway.

You will be working closely with MDT colleagues to develop the Frailty pathway across our in-patient service, to prevent admissions and facilitate early discharge. You will be committed to joint working with our community teams to provide a care closer to home approach to discharge planning and supervising the development of our Therapy Assistant Practitioners across the team.

In the future, this post will form part of a Band 6 rotational programme within Occupational Therapy as this develops.

Professional Qualification to degree level or equivalent in Physiotherapy/Occupational Therapy HCPC Registration Post Graduate specialist qualification/significant postgraduate experience

Skills, Knowledge & Abilities

Broad and specialist knowledge of relevant patient conditions and therapy interventions Demonstration of knowledge of core and specialist Therapy skills Evidence of ongoing CPD in relevant area Experience of multi-disciplinary communication, assessments, and record-keeping Computer Skills including word processing and e-mail
