Within this role, you will work closely with the MDT based on the rehabilitation unit on ward 24. You will work within an integrated model of care to provide timely assessments, interventions, and support with discharge planning for our patients. You will provide support to junior members of the team performing specialist assessment/treatment of patients admitted through the acute medical pathway to facilitate their ongoing management and discharge into community services. You will gain experience of working in a busy and pro-active environment where evidence-based practice is embedded and joint working, service development, and CPD is encouraged. This is an ideal opportunity for an experienced band 5 looking to progress in their career or an existing band 6 wanting to specialise in this dynamic clinical area. You will have gained experience as a band 5 and have evidence of relevant skills required for a band 6 post in a range of clinical areas. As a band 6, you will receive support from the Band 7 Therapy team leader and peers. You will be expected to provide supervision to more junior staff and students. The service operates over 6 days and is working towards a 7 day model; therefore, there is an expectation to be flexible in terms of working patterns. Some weekend working and bank holidays are required as part of the role and extra hours if necessary.
