The post holder will provide professional and clinical leadership for Specialist Palliative care and end of life care within the North and East locality area. To support service improvement for Specialist Palliative Care and End of Life Care services, to manage a team of nurse specialists and lead by example. Acting as a resource for Specialist Palliative Care and End of Life Care within the locality for other health and social care professionals. In partnership with senior managers and End of life facilitator, the post holder will ensure robust systems are in place to monitor and evaluate implementation of guidance, policy and strategy for Specialist Palliative care and End of Life care. The post will be based at Liskeard community hospital primarily, but applicants will be expected to travel and spend time at other locations around the county for meetings and other events. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements. Proposed interview date: 28th February 2025 Main duties of the job

The post holder will manage a geographically defined caseload using recognised referral guidelines and agreed levels of intervention to ensure that all caseload patients with complex palliative care needs receive high quality symptom control assessments, psychological support and advice to meet their individual needs. Community Clinical Nurse Specialists work as part of a multi-disciplinary team and work closely with other Specialist Palliative Care Providers. The post holder will participate in a rota providing a seven-day service. They will demonstrate high levels of clinical decision making, monitoring and improving standards of care through supervision of practice, clinical audit, research, teaching and by providing support to professional colleagues. Education / Qualifications and Relevant Experience

Current professional registration - Registered Nurse Evidence of further formal study, eg. Post Graduate MSc qualification/module, relevant palliative care course Evidence of broad range of senior experience in palliative care High level of skill and experience including supervising people including qualified and support staff Knowledge

Specialist knowledge and application of specialist palliative assessments and interventions Disclosure and Barring Service Check
